Community Provision Survey

Warwickshire Community Provision for Working Age Adults with Disabilities and/or Mental Health Needs.

Commissioners are currently reviewing community services in Warwickshire for working age adults (16-64 years) with Disabilities and/or Mental Health needs.

There are several services that are involved in this which include Day Opportunities, Complex Needs Service, Supported living, Live in Care, Respite and Short Breaks.

Whilst looking at how these will be delivered in the future it is important that they gather your views to help them design these services to best meet needs.
